The obverse of the medal depicts a profile portrait of Alexander III. Along the rim is the inscription B.M. ALEXANDER III EMPEROR AND SELF-MODERNESS OF ALL RUSSIA. On the reverse is a four line inscription: FOR EXCELLENT POLICE SERVICE. The rim of the medal on both sides is decorated with a solid laurel wreath, bound at the top with a ribbon.
